Standard Referral Fee - These terms will apply if this box or no box is checked:

For any opportunity that is converted into an active DIYLife policy, we undertake to pay you an ‘introduction fee’ equivalent to the first 3 months premiums. (An active policy is a policy for which the DIYLife insurance provider receives payment and which bears your VIP Code).

Donation - An amount equal to the standard referral fee is paid to Habitat for Humanity

No Referral Fee – No introduction fee will be payable.

Appointment Get Smart Financial Solutions (GSFS) referred to as “we”, “us” and “our” appoints “You” ( the accounting or legal firm detail in this registration) as an authorized introducer. This means your clients have the opportunity to purchase DIYLife insurance products via the DIYLIfe website using your allocated VIP Code. When your fee (if any) is paid We will pay you the introduction fee on a quarterly basis (March 31st, June 30th, September 30th December 31st) for all new active policies established in the prior quarter using your firms unique VIP Code. Conditions under which your fee is refundable to us We seek policies that are persistent. Based on this assumption, we pay your introduction fee up-front, in good faith. Should a policy lapse within the first six months of inception we’ll be entitled to receive a full refund from you for the amount paid. Termination of this agreement This agreement can be terminated by either party giving the other 1 month’s notice that it intends to cease operating under its terms. Please note that you remain liable for any fees that become refundable to us if policies lapse in the six month period following the termination.
